DIAGNOSIS
The first step is to make an accurate diagnosis of the teeth to be treated. This includes taking X-rays, photographs, and models to obtain an accurate color match and using a palatal silicone key to restore the lost shape.
TREATMENT WITH DENTAL COMPOSITE
In the next step, absolute isolation is achieved using a rubber dam, and any previously affected restorations due to cavities are removed.
Before the photographic study, color analysis is performed to determine the stratification of successive layers of composite (Kiyomi’s Aster Dental Composite). Following a biomimetic approach, enamel is used in the palatal structure, dentin in the tooth body, and enamel on the vestibular layer. In this case, an A2 enamel layer, A2 dentin, and an outer enamel layer are used.
FINISHING AND POLISHING
For finishing and polishing, the process begins with macrotexture using fine diamond or tungsten carbide burs to remove excess material and redefine the shape. Then, fine-grit softlex discs are used to correct interincisal angles, incisal edge length, and transition lines. Additionally, coarse-grit burs are used to create perikymata, which are developmental lines on the tooth’s surface that influence how light is reflected on the vestibular surface.
Finally, polishing pastes with progressively smaller particle sizes are used to achieve the final shine.
